“For when I kept silent, my bones wasted away through my groaning all day long. For day and night your hand was heavy upon me; my strength was dried up as by the heat of summer. Selah” -Psalm 32:3–4
In these verses, David describes the turmoil of the unrepentant. Having kept silent about his sin, that is to refuse to confess his sins to God—he was wasting away under the heavy hand of God. This picture of stubbornness is so common among humanity, I would opine that many of the physical and mental maladies people carry with them—anxiety, etc.—are a result of unconfessed sin.
David will deal with his sin in the next verse. But if the condition described here is not familiar to us, then let it be a warning that God will not let his children abide in a state of unrepentance. His heavy hand that dries up our joy and peace is really the loving hand of a Father who desires to end the estrangement.
